Office of Rural Health Policy Workshop on Decline of General Surgeons in Rural America

The Office of Rural Health Policy is convening a workgroup for a one-day meeting (probably in DC) to look at the issue of the decline of general surgeons in rural America and its impact on healthcare in rural areas. NASEMSO was invited to send a representative to the meeting. The shortage of not only general surgeons, but even specialty physicians in tertiary trauma centers, has been of great concern. It seems all rural states are having challenges to effecting timely inter-facility transfers.

NASEMSO has been invited to join the Public Safety Alliance, the group pushing to have the D Block allocated to public safety. NASEMSO is the only EMS group being asked to join, largely because of an earlier effort with the letter for the eight EMS associations.

Public Safety Spectrum Trust (PSST)

Kevin McGinnis, representing NASEMSO, was elected Vice Chairman of the Public Safety Spectrum Trust (PSST) in Oct. 2007. Among the short-term goals for the PSST is the preparation of a Statement of Requirements (SOR) defining the public safety network for interested commercial partners. The SOR is planned to be released in advance of the 700 MHz auction short form application deadline. This document will serve as a general blueprint for the construction, operation and cost of the public safety network. Its purpose is to ensure potential 700 MHz D Block bidders are appropriately informed before the spectrum auction begins on Jan. 16, 2008. Following requirements provided by the Federal Communications Commission (FCC) in its Second Report & Order on the 700 MHz band released on Aug. 10, 2007, and its Sept. 24, 2007, Order on Reconsideration, the PSST is also now preparing its application to the FCC for the position of national Public Safety Broadband Licensee (PSBL).The PSST Board of Directors is comprised of representatives of the following organizations: the American Association of State Highway and Transportation Officials (AASHTO); the Association of Public-Safety Communications Officials-International (APCO); the Forestry Conservation Communications Association (FCCA); the International Association of Chiefs of Police (IACP); the International Association of Fire Chiefs (IAFC); the International Municipal Signal Association (IMSA); the National Association of State Emergency Medical Services Officials (NASEMSO); the National Emergency Number Association (NENA); and the National Sheriffs' Association (NSA). In addition to these organizations, the International City/County Management Association (ICMA) and the National Governors Association (NGA) have been invited to name representatives to the Board, and the FCC will be appointing four separate organizations as part of the board of the PSST. Rural Domestic Preparedness Consortium (RDPC) Advisory Board

The nation’s rural emergency responders face unique challenges when compared to their urban counterparts. In recognizing the need for consistent, quality training which addresses those challenges, Congress and the Department of Homeland Security (DHS) established the Rural Domestic Preparedness Consortium (RDPC or the Consortium) to develop and deliver relevant all-hazards training in support of rural homeland security requirements. NASEMSO is represented on the RDPC Advisory Board. The RPDC point of contact is Project Administrator Leah Baldwin, 859-622-6861.

The Society of Chest Pain Centers (SCPC) is a non-profit international society that bridges cardiology, emergency medicine and other professions focused upon improving care for patients with acute coronary syndromes and acute heart failure. The SCPC has commissioned an EMS Task Force with the specific objective of promoting good prehospital care of patients with ACS. The e-mail discussions thus far have included: working with state medical directors to improve state protocols (raise the bar across the country); working with CMS to improve reimbursement for assessing chest pain patients (12 leads to rule out STEMI coded at the ALS 2 Level); reviewing EMD protocol supporting the inclusion of ASA (currently an option); and developing a "STEMI Credentialing" for paramedics, ER nurses, etc. The intent is that if a paramedic is credentialed, the recognized STEMI receiving hospitals would "activate" the cath lab and cardiologist based on the paramedic's "read" of the EKG and clinical condition of the patient.

The State and Territorial Injury Prevention Directors Association (STIPDA) is a national non-profit 501(c)3 organization of professionals committed to strengthening the ability of state, territorial and local health departments to reduce death and disability associated with injury and violence. To advance this mission, STIPDA engages in activities to: increase awareness of injury including violence as a public health problem; provide injury and violence prevention and control education, training, and professional development for those within the injury and violence prevention field; enhance the capacity of public health agencies to conduct injury and violence prevention programs; and support public health policies designed to advance injury and violence prevention.

The ITS America Transportation Safety Advancement Group (TSAG) advises the U.S. Department of Transportation on development and deployment of ITS technologies optimizing travel mobility, safety and security, economy, and environmental quality. Formerly known as the Public Safety Advisory Group (PSAG), TSAG continues its charter mission of advancing the application of transportation technologies for enhanced transportation safety, mobility, and economy. TSAG is comprised of members from key transportation and public safety interests and stakeholders: fire and rescue; emergency communications; law enforcement; EMS; transportation operations; emergency management; technology and telematics; and academics and research. Under its new name, TSAG continues to pursue an aggressive agenda to gain insight from public and private interests for providing feedback and guidance to the ITS Joint Program Office of the U.S. Department of Transportation on policy and program matters relating to proven and emerging transportation technologies.

This Project Advisory Group (PAG) will advise the ESAR-VHP program on the development of a single national interoperable network of State-based ESAR-VHP systems, as required by the Pandemic All-Hazards and Preparedness Act of 1996. The PAG will serve as a steering committee for the project. Its purpose is to ensure all parties impacted by this effort are represented; that expert opinion and experience is obtained; that a wide range of options for developing an interoperable network are developed, and that a comprehensive, reasonable and attainable plan is proposed. PAG members will also help address a number of issues identified by States, ESAR-VHP system vendors, and others. Issues to be addressed include the process for requesting and responding to requests for volunteers; data standards and data security; technical variations in State systems; financial costs; and others. The PAG will also provide comments on the final recommended approach for developing an interoperable network that all States can use when exchanging volunteer health professionals. PAG members from State ESAR-VHP programs may also be asked to participate in a proof of concept demonstration of the approved approach for developing an interoperable network.

U.S. Department of Homeland Security "Blue Campaign" to Combat Human Trafficking

In Summer 2010, the U.S. Department of Homeland Security (DHS) launched the Blue Campaign — a collaborative "One DHS" effort to combat human trafficking through prevention (enhancing public awareness), protection (increasing victim assistance programs), and prosecution (law enforcement training and support). NASEMSO was invited to participate in discussing ways in which the DHS Blue Campaign can better engage the medical and EMS communities in this effort.
